As responsible NDIS providers, we at Clare Health and Well-being document serious incidents affecting participants. If you are a support worker, you can report any and every incident and help us maintain our records.
We are committed to creating a safe and supportive workplace where individuals feel valued. So, we encourage you to report incidents that can benefit us and you in the long-term.
Importance of Incidence Reporting
Reporting an incidence is important due to various factors. It helps us
- Understand the problem(s) in detail
- Gain insights into the challenges and other potential risks
- Create a culture based on improvement and knowledge-sharing
- Create better processes, procedures and safety measures
- Ensuring a safer workplace that benefits all staff and participants

Incidents That You Should Report
A few examples of incidents that you should report include:-
Serious Incidents resulting in harm or potential harm for all disability participants
- Tracking the behaviour of participants
- Staff injury
- Vehicle damage
- Property damage
- Privacy breach
- Reporting an OHS hazard
